Huge congratulations to our specialist orthodontist Dr Sunil Hirani for recently passing his World Society of Lingual Orthodontics (WSLO) exam.

Dr Hirani has been busy preparing for this prestigious qualification. The majority of specialists sitting the examination have practised lingual orthodontics for a minimum of 10 years.

We are extremely proud that Dr Hirani has achieved this accreditation.  He is currently the only consultant-trained specialist orthodontist in the UK to hold this qualification. An expert panel of lingual examiners from around the world assessed his cases, considering difficulty, standard of records, treatment planning skills, efficiency of treatment, outcome, cephalometric tracings and post-treatment stability. As of 2019, only 246 individuals have passed this exam worldwide so it’s a huge achievement.

The examination took place at the 8th WSLO Congress in Barcelona, Spain.  Dr Hirani was also an invited speaker and gave a presentation on Lingual lever arm mechanics and the current evidence.  This was extremely well received.

Lingual braces are braces that are fixed to the back of your teeth so they are very discreet and almost invisible.  We are very fortunate that we are able to offer this treatment to our patients here at SmileLux under the skilled and expert guidance of Dr Hirani. Dr Hirani is also the chairman of the British Lingual Orthodontic Society (BLOS).

Different types of Invisalign treatments

What is Invisalign?

Invisalign is a very different style of orthodontic treatment from the traditional fixed braces. It is a fully bespoke, removable brace system where fabricated trays (known as aligners) are made for you. The greatest benefit of this treatment is that it is virtually invisible therefore nobody need even know you are having treatment!

You wear each set of aligners for two weeks and then change to the next set. Each set is very slightly different and a gradual movement occurs where your teeth are able to move into their ideal position. Before you decide to proceed with the treatment, moulds are taken and sent to the Invisalign laboratory. They create a computerised simulation that will show you what your teeth will look like after treatment and will also determine how many aligners you will need. Each aligner is changed every two weeks; therefore the simulation will give you a guide as to how long your treatment is expected to take. This is usually determined by the complexity of the treatment and the results you and your orthodontist are hoping to achieve.

Invisalign should be worn for a minimum of 22 hours per day to achieve the very best clinical results and to ensure that your treatment runs smoothly and promptly. You must remove the aligners when you eat and drink anything other than cold water as the aligners are delicate and can be easily damaged. It is very important to maintain good oral hygiene and keep both your teeth and the aligners very clean to prevent dental problems arising such as tooth decay or plaque.

The different types of Invisalign treatment ranges are listed below:

Invisalign full

Most standard treatments would fall into this category where more than 14 aligners are required. Treatment time can vary from 28 weeks to 2-3 years+ for the more complex cases.

 

Invisalign Teen

Invisalign is available for teenagers between the ages of 12 and 19. It works in the same way as Invisalign full but has added benefits for parents such as blue indicators at the back of the aligners that fades from blue to clear. This helps parents and the orthodontist easily check if the aligners are being worn as they should.

 

Invisalign Lite

This is designed for less complex cases requiring no more than 14 aligners. This means that treatment time is shorter and can often be completed within 6 months. It is also possible to just treat the upper or lower teeth alone. They are generally suitable for patients who have minor orthodontic issues such as slight overcrowding or small gaps between the teeth.

 

Invisalign I7

Ideal for extremely mild cases requiring less than 7 aligners. This means treatment time can be as little as just 3 months. Invisalign I7 generally only moves the front 6 teeth.

14 things you should know about Invisalign

1. Who should I see for my Invisalign treatment

It is important to do your research and make sure you feel comfortable with the dentist who will be treating you, the clinic and the team who will be looking after you. You will all be working closely together for several months so it is important that you have confidence in the team. Any dentist can treat patients using Invisalign, however an orthodontist specialises in moving teeth with braces and has experience in working with a large range of orthodontic systems which can help, especially if your case is complex. Remember, all orthodontists are dentists; not all dentists are orthodontists.

2. The fees

The fees will vary depending on your individual case and needs however as a guide the investment can range between £3000 and £4500 which is generally based on 12-18 months of treatment. For very complex cases this can be more and there are also simpler treatments for very mild cases that can range from just £1500. 0% interest free finance is available to help spread the investment over up to 18 months and make treatment more affordable.

3. Will Invisalign work for me?

As with any brace, for Invisalign to work you would need to adjust your lifestyle. The aligners must be worn for no less than 22 hours per day and you must remove them when you eat or drink anything other than cold water. If you are not strict with wearing the aligners, you may be faced with your treatment taking longer than planned or not achieving the best result possible. Most cases are suitable for Invisalign these days, however there are some cases where only a fixed brace will provide the best result. The great thing about Invisalign is that you can see what the end result should look like before you actually commit to the treatment. This is known as a ClinCheck (3D simulation) which is charged at £350 and is usually deducted from the final balance of the Invisalign treatment fee.

4. How long should each aligner be worn for?

The aligners should be worn for 2 weeks before changing to the next set. Moving teeth should be done at a slow and gentle pace as moving teeth too quickly can cause shrinkage of the roots which can make teeth less stable and become mobile over time.

5. What do the aligners feel like in the mouth?

As movement is fairly gradual with Invisalign, the treatment is generally pain free. You may notice slight tenderness when you change to the next set of aligners but this is a sign that the teeth are moving as they should be. It will take a few days to adjust to wearing the aligners and adapting to a new routine, as you would expect with anything. Many patients miss their aligners when they finish treatment.

6. Will people notice them?

Although the aligners are clear and virtually invisible, you may find that people notice something different about you but may not be able to pin point what it is. The aligners can make your teeth appear shinier and may discolour very slightly as you are getting close to your next aligner change. Many of our patients get through treatment and tell us no-one even knew.

7. Your love life and Invisalign

Life can carry on the same while wearing Invisalign and patients have no problem dating or kissing while wearing the aligners. It is important to keep your teeth and the aligners clean and you can even purchase cleaning tablets that dissolve in water to soak them regularly so they stay nice and fresh.

8. Will Invisalign cause me to speak differently?

There may be temporary change to your speech initially while getting used to your new aligners, however this subsides after a few days once your tongue gets used to them being there.

9. What are Invisalign attachments?

Invisalign attachments are a necessary part of Invisalign treatment. They allow the aligners to get more grip in certain areas where teeth may need more complex movement. We use tooth coloured composite that is removed after your treatment and as long as you are wearing your aligners as instructed, they are virtually invisible.

10. What is Interproximal Reduction?

In modern day orthodontics, we try and avoid removing healthy teeth to create space for teeth to be realigned. Where possible, we use a technique called “Interproximal reduction” or “Interproximal stripping”. We use a special technique that carefully and accurately removes a fraction of a millimetre from your enamel. This procedure is safe in skilled and experienced hands such as Dr Hirani’s. It allows us to make sufficient room to tuck your teeth in line and make them straighter. It is not harmful to the teeth in any way and the procedure itself is not in any way painful.

11. How to keep your aligners clean?

The best way to clean your aligners is with a very soft toothbrush and a small amount of your daily toothpaste. You can also purchase cleaning tablets that dissolve in water to use occasionally if you feel that your aligners need freshening up.

12. Eating and drinking during Invisalign

The main benefit of Invisalign is that you can continue eating all the foods you enjoy and do not need to make any changes in your diet. You may need to change your lifestyle and the times of day you eat, drink and snack. You must never wear your aligners while eating or drinking anything other than cold water as they can become distorted easily with heat. Anything that contains sugar or acid can sit on your teeth beneath the aligners and will prevent your own natural saliva, that fights bacteria throughout the day, reaching your teeth which can cause plaque or even decay over time. You will also need to clean your teeth and your aligners after every snack, drink or meal.

13. How often will I need to visit the orthodontist?

You will need to attend around every 6-8 weeks to check your progress and be given more aligners. It is normal to be given 3-4 sets at a time. You may also require interproximal reduction at certain intervals throughout your treatment and you will be notified by the orthodontist.

14. Will I need to wear retainers

After any orthodontic treatment, retainers are essential to prevent all your hard work coming undone and your teeth moving again. You will need to wear retainers for life, permanently. We recommend and fit 2 different types of retainers for our patients as standard. One is a bonded or fixed retainer which consists of a thin wire that is secured behind your front 6 or 8 teeth. The other is a removable “bedtime” retainer that is similar in appearance to your Invisalign aligners and is worn for 2 nights per week in conjunction with a bonded retainer.

How to get the best results from your Invisalign treatment

If you’re straightening your teeth with invisible braces such as Invisalign, you’re probably excited about getting to the end of your treatment and celebrating your straight smile. To ensure you get the best results from Invisalign it is important that you follow the advice given by your orthodontist.

Invisalign involves a series of clear and removable aligners which must be worn for 14 days before moving onto the next set. It works by gently guiding the teeth into the desired position using low forces, so you shouldn’t feel any pain, although you may experience some discomfort for the first day or two as your teeth adapt to wearing a new aligner.

For optimum results it is recommended that you wear the Invisalign aligners for 22 hours a day. If the aligners are not worn for this desired length of time each day, the treatment can take longer and the overall result could be affected.  It is very important that you take the aligners out of your mouth while eating or drinking foods as this can cause discolouration or even damage the thermoplastic material which the aligners are made out of.

Keeping your Invisalign aligners clean is essential. You can do this by brushing them with a little fluoride toothpaste and water after you have cleaned your teeth and before the aligner is put back in your mouth. There is a product called Retainer Brite® that can be purchased to soak your aligners in if they do begin to look a little grubby.
